can be accessed here. Print this form and complete and attach all necessary information and bring to college office for processing. Please remember to allow THREE WEEKS for processing. You will need to print a new form for each school with the exception of CUNY and SUNY. You can include all CUNY schools on a single form and all SUNY schools on a single form.
